Understanding PSA
What Is PSA?
PSA stands for Prostate-Specific Antigen. It is a protein produced by both normal and cancerous prostate cells. Small amounts of PSA normally enter the bloodstream, and a simple blood test can measure the level of PSA in your blood.
PSA is measured in nanograms per milliliter (ng/mL). There is no single 'normal' PSA level — what matters is the number in context of your age, prostate size, symptoms, and how the number has changed over time.
What Can Cause an Elevated PSA?
An elevated PSA does not automatically mean cancer. Many conditions can raise PSA levels, including:
- Benign prostatic hyperplasia (BPH): A non-cancerous enlargement of the prostate, very common in older men.
- Prostatitis: Inflammation or infection of the prostate.
- Recent ejaculation: Can temporarily raise PSA. Many providers recommend abstaining 48 hours before a PSA test.
- Prostate biopsy or procedures: Can significantly elevate PSA for weeks.
- Prostate cancer: Cancerous cells tend to produce more PSA than normal cells.
PSA is a screening tool, not a definitive test. A high PSA prompts further investigation — it does not confirm cancer. Likewise, a normal PSA does not guarantee the absence of cancer.
PSA Levels and General Ranges
While there is no universally agreed-upon cutoff, many providers use the following general reference ranges. These are guidelines only — your doctor will interpret your PSA in the context of your full clinical picture.

Note: Age-adjusted PSA ranges are sometimes used. Younger men may be evaluated at lower thresholds.
PSA Density and PSA Velocity
Your doctor may also consider two related concepts:
- PSA density: Your PSA level divided by the volume of your prostate (measured by ultrasound or MRI). A higher PSA density suggests the PSA elevation is more likely due to cancer than to a large but benign prostate.
- PSA velocity (PSA kinetics): How quickly your PSA is rising over time. A rapid rise — even within the 'normal' range — can be a warning sign that warrants closer attention.
PSA After Treatment
PSA is also used to monitor your response to treatment and to watch for signs of recurrence:
- After surgery (radical prostatectomy): PSA should drop to an undetectable level (typically below 0.1 ng/mL). A rising PSA after surgery — called biochemical recurrence — may indicate remaining or returning cancer.
- After radiation therapy: PSA typically falls gradually over 18–24 months to a nadir (lowest point). A rise of 2.0 ng/mL above the nadir is the standard definition of recurrence after radiation.
- During active surveillance: PSA is monitored regularly (often every 3–6 months) to detect any significant changes that might prompt re-evaluation.
Keep a personal record of your PSA results with dates. Trends over time are often more meaningful than any single number. Ask your provider for a copy of each result.
It's normal to feel anxious while waiting for PSA results. If you tend to worry, schedule your blood draw a few days before your follow-up appointment whenever possible. Reviewing the results with your healthcare team right away can help prevent unnecessary stress and unanswered questions.
